DATA COLLECTION & CONSENT POLICY

You will have navigated to this page from the link with our Service Level Agreements. Therefore, you will have chosen Your Therapy to provide an Allied Health Service under the National Disability Insurance Scheme(NDIS).

In order to do so, we must collect your relevant personal information. The collection of personal and health information requires Your Therapy to obtain informed consent from the you.

How will my information be used?
The information that you provide about your personal and medical history will be used to assist Your Therapy staff in completing the relevant Allied Health Service that you have engaged us to perform.

As you are a participant of the NDIS, this means that it may be necessary for Your Therapy to share information about you, your assessment and your plan as required by the NDIS. If you have chosen to engage a Support Coordinator, Plan Manager or Nominee, we may also be required to share the aforementioned information with these stakeholders.

Information that you provide to us will be recorded and stored securely. We understand that some information may be sensitive, and we will keep your information confidential. The only exception to this is if you tell us anything indicating that someone might be in danger; then we have a responsibility to act on that information, but we would not do this without telling you and including you in the process.

Your Consent

By confirming your consent in our Service Level Agreement, you confirm that: 

You have read the information above and understand that Your Therapy require the collection of your relevant personal information. You understand how your personal information will be used and stored. You understand that your personal information will only be shared with the nominated stakeholders involved in your care and NDIS plan. You understand that if any information collected raises safeguarding concerns relating to yourself or another person. Your Therapy are obliged to escalate this to external parties however will keep you informed at all times.
